Class Application Forms

You can register for our classes on-line with the specific classes on our Class & Event Schedule Page. Or you can print off the following forms and mail them in. Please use these Adobe Acrobat documents to complete the Group Training applications for any class you are interested in. The following is a brief description of each of the classes and the form(s) for each.

The classes listed below contain links to whatever paperwork applies to each specific class. This may be an application, helpful class information for you to know before you come to class, or a questionnaire to help us best meet your needs and desires. If you have any questions, contact us.

Puppy Pre-School
This is a simple application to give us a little more information about your puppy.

In addition to our Level 2 More Manners and Beginner 2 Agility, this application is for our Specialty Classes such as Obedience for Agility, Loose Leash Walking, Rally, Dance, and Advanced Agility. If you've enrolled in one of our advanced Manners Classes, such as Level 2 or Level 3, please fill out the second page of the application, which is a Behaviors Questionnaire.

Practice in different locations, in real-life situations. Application package includes a Behavior Questionnaire.
